Disable username & password login prompt when SSO / SAML is enabled

If SSO or SAML is enabled, the ability to hide the username & password prompt would be useful

The value in SSO is not so much that it's easier to login, but also that we can prevent people logging in via their password
so if they leave the company, we can disable their account in one place

There is now a setting to prevent non-SSO sign in for your company.
